If this is your pet’s first visit or if you are an existing client with a new pet, you will have some paperwork to fill out. If you wish, you can print our new Client / Patient form and bring it at the time of your pet’s examination.
Please telephone ahead for an appointment so we may maximize our efficiency and minimize your waiting time. Before your first meeting with Forestville Veterinary staff and doctors, please have all your pet’s previous medical records and /or adoption information ready. Please, bring your pet in a carrier or on a leash. You will be greeted by a patient’s advocate and a technician. He will get a weight of your pet, take his/her temperature and a small history about the reason of your visit. The doctor will then listen to you then ask some questions before performing a full physical examination on your pet. At the end of the physical examination, the doctor will discuss your pet’s health condition then recommend and discuss any appropriate test or procedure. |
